Hometown Heroes Banner Program Disclaimer

  1. Give the City of Coral Springs permission to release the information for public use, including the name/branch/rank/dates of service and photograph.

  2. Certify that all statements made in this application are true and complete. Permission is granted to the City of Coral Springs to investigate and verify criminal history and any information provided on this and successive documents completed for purposes of my participation in this program. In return for my consideration of my application, I release any person who provides information pertaining to me from all claims or liabilities that might otherwise result from such information or opinions. A criminal history may disqualify an applicant from participation in the City’s Hometown Heroes Banner Program.

  3. In consideration of the City of Coral Springs granting me permission to participate in the Hometown Heroes Banner Program, I agree to indemnify, defend, save and hold the city, its officers, agents and employees, harmless from any and all claims, damages, liability, losses, causes of action of any nature whatsoever, which may arise out of, in connection with my application and/or participation in the City’s Hometown Heroes Banner Program.

  4. The city has full editorial control over the location, duration, placement, content, appearance and wording for the banners and promotional aspects of this program. City is not responsible for lost/stolen banners, or repairs to the banners should they be damaged. Quantity is limited to one banner per service person. Active-duty status and program eligibility will be verified. The spelling of the name, rank, dates of service and branch will be taken directly from this application.

  5. Qualifying applicants with a complete and approved application shall be recognized on a first come, first-served basis.

  6. Electronic Signature Agreement Acknowledge that 1) your application will not be signed in the sense of a traditional paper document, 2) by signing in this alternate manner, you authorize your electronic signature to be valid and binding upon you to the same force and effect as a handwritten signature, and 3) you may still be required to provide a traditional signature at a later date.
